Renewal Fees:
In person: $40(cash) 43.15(card)
Video Visit: $50(online invoice)
Renewing just got a whole lot easier with access to your medical provider with virtual video visits and electronic application submissions- renewing your medical card benefits just got easier.
Renewal appointments must be made within 90 days of the current calendar year date. State Department of health recommends no later than 30 days and no greater than 90 days.

This full evaluation option is for PTSD only at this time. This appointment does not require previous medical records and will be determined based on the diagnostic criteria of DSM-V. Only patients who meet the criteria of this condition may receive cannabis recommendations if benefits away the risks of cannabis use.
